- Effective Behavior Management (7 items, section C items 1-7)
- Positive Attention (2 items, section D items 1-2)
- Student's Positive Behavior (7 items, section E items 1-4, 6-8)
- Quality of School Common Area (4 items, section G items 1-4) - assessed in gym, gameroom, or outdoor activity area only
- Student's Problem Behavior (11 items, section F, no scale created because of low base rates for many of the behaviors—the highly antisocial ones have no variance)
